Voluntary Dental Insurance Plan
Gunnison Valley Health provides you the option to purchase a voluntary dental insurance plan through Lincoln Financial. The plan offers in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will pay less out of your pocket when you choose a network provider. If a non-network provider is used, expenses are reimbursed based on reasonable and customary (R&C) charges. Any charges over the R&C charge will be your responsibility. Voluntary Dental Discount Plans
Gunnison Valley Health provides you the option to purchase two voluntary dental discount plans through Beta Health Association. Each plan is a discount fee-for-service (not insurance) dental plan and offers in-network benefits only. Locate an Alpha or Blue Expert network provider at www.betadental.com; select ‘Dentist Locater’ from ‘Dental Providers’. You must choose a provider off of the provider list and state your provider when enrolling in benefits.

Vision Discount Plan
Gunnison Valley Health offers a vision discount plan at no cost to all eligible employees and their dependents through EyeMed. This plan provides unlimited point-of-service discounts for exams, frames, lenses, and contact lenses. To access these discounts, please see Human Resources for an ID card.
